ADDITIONAL INFORMATION - IMPORTANT FEDERAL MANDATE REGARDING MEDICARE DISCLOSURE

We previously provided you with information about employer requirements to notify employees and their dependents who are Medicare-eligible as to whether your prescription drug plan was "creditable".  Employer groups are also required to notify CMS (The Center for Medicare and Medicaid Services) whether their prescription drug coverage qualifies as creditable or non-creditable. 

The disclosure to CMS is due no later than sixty (60) days following the beginning of the employer group's plan year (e.g., renewal year, contract year, filing year, etc.)  Employer groups are required to electronically complete and submit the disclosure form found on the CMS web site at:

CMS is operating in a paperless environment; only electronic filing is available and employers who do not have internet access are urged to seek alternative internet access.
